Media Requirements

Nominations for this award are made by service users, carers, their families, staff and partner organisations.

Candidates are teams, individuals or a service.

They need to have demonstrated that they have put the person at the centre of the care, focussing not only on their mental and physical health but also their social needs.
Alternatively they may have been instrumental in breaking down the barriers between services and /or /organisations by developing exceptional and effective partnerships to deliver all round care.
